Community Wellness Screenings

Crawford County Memorial Hospital is committed to the health and safety of all of our patients. We encourage you to get the appropriate health screenings rather than putting off needed care. Please use the main entrance when arriving for your wellness screening. Thank you for your help and cooperation in making the Community Wellness Screening a safe and successful event!

PLEASE NOTE: It is the patient’s responsibility to make a follow-up appointment and share the results of your wellness screening with a primary care provider. This will ensure proper interpretation, discussion, and follow-up for any abnormal laboratory test results.

CCMH is now offering quarterly Wellness Screenings!

These screenings will be held at Crawford County Memorial Hospital in Meeting Rooms C & D on the times listed below.
  • Tuesday, September 16, 2025 from 6:00am to 8:00am
  • Wednesday, September 17, 2025 from 6:00am to 8:00am

How Much Does It Cost?

Basic Wellness Screening

The Basic Screening Blood Tests Include:

CBC

Complete Blood Count

Lipid Panel

Total Cholesterol, Triglycerides, HDL & LDL

Comprehensive Metabolic Panel

Made up of 14 tests that measure your blood sugar level, electrolyte and fluid balance, kidney function and liver function.

Thyroid Function Screening

Note: If receiving a TSH screening, please stop using all high-dose, biotin-containing supplements for hair, skin, and nails at least 5 days prior to your appointment. High doses of biotin can falsely affect TSH test results.

For those who have been diagnosed as diabetic, or pre-diabetic, you can add an A1C blood sugar test.

Note: You must have a diagnosis of diabetes or pre-diabetes to receive an A1C screening test.

Screenings are not covered by insurance and fees must be paid at the time of service. Cash, checks, debit and credit cards accepted.
